Job Description:

This position is responsible for effectively and safely dispensing medications and other pharmaceutical supplies; in order to fill medication orders issued by physicians and other qualified persons. Provide guidance and knowledge regarding drug information for all other health and medical professionals. MAJOR RESPONSIBILITIES: Prioritize and enter medication orders accurately and in a timely manner Practice in accordance with federal, state and local laws and in compliance with the requirements of the Texas State Board of Pharmacy and Seton regulations and code of conduct Abides by all regulations and policies for Controlled Substances at all times Oversees technical personnel who assist in the preparation, maintenance, and dispensing of medications and supplies Coordinates and supervises drug distribution, including outpatient prescriptions, investigational drugs and sterile products Review individual patient medication orders for accuracy, safety and effectiveness and takes corrective action as indicated Promote cost effective drug therapy in accordance with departmental initiatives and policies Consistently completes all pharmacy initiatives including, but not limited to, dose optimization, therapeutic interchange, ADR reporting, anti-coagulation monitoring, vancomycin and amino glycoside monitoring, target drug monitoring, and drug use evaluation Provide patient counseling efficiently and effectively when required Properly documents all clinical interventions and other information as required by law and departmental policies and procedures Assist with academic functions such as serving as a preceptor for pharmacy students A. Education/Skills/Experience: Doctor of Pharmacy degree Current Pharmacist license from Texas State Board of Pharmacy is required Prefer hospital experience to include automated order entry skills, IV admixture service to include chemotherapy and TPN compounding, neonatal order entry is a plus, and experience in clinical dosing of antibiotics and anticoagulation monitoring Good leadership skills are required Ability to communicate effectively with physicians and other allied health professionals is required B. Licenses, Registrations, or Certifications: Current Pharmacist license from Texas State Board of Pharmacy BLS through the American Heart Association
