Job Title: RN - Case Management Profession: RN Specialty: Case Management Duration: 13 Weeks Shift: Monday - Friday Hours per Shift: 8:00 AM - 5:00 PM Experience: Minimum 1 Year License: RN License Required Certifications: None Required Must-Have: Candidates must have bedside case management experience. Candidates must have at least 1 year of post-acute care knowledge. Candidates must have experience with Meditech or HCA. Description: This is a contract position for an RN in Case Management. The schedule is Monday to Friday from 8:00 AM to 5:00 PM. No on-call duties are required for this role. The unit profile includes a varied number of beds and staff members. The patient ratio is 1 RN to 30 patients. The staff mix includes 15 RNs and 15 social workers on day shift. The role involves discharge planning and progression. The position requires having difficult conversations with patients and families. The RN will engage in open discussions with the medical team regarding care plans. Candidates should have a strong personality for having challenging discussions. Adaptability and proactivity are key traits for success in this role. The dress code consists of navy scrubs with a white lab coat. Business casual attire is also acceptable, with closed-toe shoes. Certain items are not permitted, including crocs and artificial nails.
